The Teaspoon: From Apothecary Drachm to Pediatric Syrup
The teaspoon as a kitchen unit emerged from English apothecary medicine in the 1700s. Apothecaries measured liquid medicines in fluid drachms (1 fluid drachm = around 3.7 mL), and the practical kitchen teaspoon became the closest household analog: large enough to hold a useful dose, small enough to fit in a teacup. By 1850, "1 teaspoonful" in pharmacist instructions meant approximately a fluid drachm; the US standardized it at 1/6 fluid ounce = 4.929 mL when its customary system was codified.
The metric system formalized the teaspoon as exactly 5 mL in the late 1800s. This was adopted by most of the world (excluding the US and UK) for both cooking and medicine. The 5 mL standard appears in every modern pediatric oral syringe, dosing cup, and pharmaceutical prescription. The US 4.929 mL tsp persists in cookbooks and casual recipes, but US pharmacy practice has migrated to mL-only labeling since FDA guidance in 2011.
The kitchen teaspoon as an informal measure has hidden risks. A 2014 JAMA Pediatrics study found that using household teaspoons for liquid medication led to dosing errors in 40% of cases, mostly under-dosing because household spoons average 4-5 mL when the prescribed dose was 5-10 mL. The American Academy of Pediatrics now recommends mL-graduated oral syringes for all liquid meds, with the syringe distributed by the pharmacy at no charge.
Cocktail bartending uses the teaspoon in two distinct ways: as a unit (5 mL) and as a tool (a long stirring spoon, the "bar spoon"). Classic recipes call for "1 bar spoon" of bitters, equivalent to around 1 dash = 0.6 mL by craft-cocktail convention. The Manhattan calls for 2 dashes Angostura; the Old Fashioned for 2-3 dashes. The bar spoon's twisted shaft (5-10 mL bowl) allows precise pouring and elegant stirring; Japanese-style mixology emphasizes bar spoon technique as much as the measure itself.
In coffee, the teaspoon is used as a rough volumetric for ground coffee. The Specialty Coffee Association ratio of 60 g coffee per liter of water (1:16.67) translates to around 7 g per cup, or 3.5 tsp of ground coffee per 6 oz brewing water. Precision pour-over brewers (Hario V60, Chemex) weigh coffee on 0.1 g scales; volumetric teaspoon measure has +/- 15% variance due to grind size and packing density. The "coffee scoop" included with most drip coffeemakers is 2 tbsp = 6 tsp = around 10 g.
WHO oral rehydration solution (ORS) field-mixing famously uses the teaspoon as the primary measure because scales are unavailable in low-resource settings. The standard recipe: 1 L clean water + 6 tsp sugar + 1/2 tsp salt. The teaspoon precision tolerance is around +/- 25%, which is acceptable for rehydration efficacy (too much salt is dangerous; too little is just less effective). Packet-form ORS, distributed by UNICEF at US$0.10 per unit, eliminates measurement entirely and saves around 1 million pediatric lives per year from diarrheal disease.
Australia's 5 mL teaspoon is the same as metric, but Australia's 20 mL tablespoon (4 tsp) breaks the universal 3 tsp = 1 tbsp ratio used everywhere else. This causes constant recipe-conversion bugs when AU recipes are followed by US/UK cooks. AU recipes calling for "2 tbsp" need 2 * 20/15 = 2.67 US tbsp. The 4-tsp Australian tablespoon also breaks pharmaceutical-conversion math. Australian medication labels use mL exclusively for this reason.
